Haynes says 45 degrees for new, so maybe 90 for used is ok

Are you sure it's firing on both? Could it be that it's firing on only one and that's why it sounds as though it's knocking??
Have you got free play when the lever is fully released? There should always be a couple of millimetres before you feel the lever start to apply...

You can always try the cable tie the lever trick. Nothing to lose..
