Rushden Graphics Co these did one for me a few year ago.......you might have to supply your own carbon but thats easy enough from ebay..

hhhjesus, do you not have a more local licence office ( i know where the nearest is to me) secondly, have you passed the 6 months period from...
i suspect that at the time the EFPN was issued the OP didn't have his licence with him. So to complete the EFPN he was required to produce both...
