Good question, maybe he declined the invitation. Instead of an honours list wouldn't it be good to have a list of those who declined. An anti honours list if you like.

^ exactly.. signed the petition years ago and despite thousands of signatures it's made no difference. I expect they'll award something when they hear he's ill - similar happened to Charlie Chaplin.
